Gums recede with age, and the root surface they expose is softer than enamel and decays faster. Root caries is common in older adults, awkward to fill because the margin sits at or below the gumline, and the filling often fails. Stopping a lesion without drilling is therefore worth a great deal, and several agents claim to do it. Silver diamine fluoride arrests decay but stains it black, which the patient sees every time they look in a mirror. Fluoride varnish and high-concentration fluoride toothpaste are gentler and less certain. Chlorhexidine varnish has been used for years on thinner evidence. The trials behind them use different concentrations on different schedules and count decay in different units, so the choice a dentist actually makes turns as much on what a patient will tolerate as on the numbers. This project takes the agents one at a time and sets out what the trials support for each, including what they say about staining and about who stops coming back. The arguing happens in week seven, when everyone reads everyone else's section.
